Home health care is the provision of health care in the patient's home. Patients feel more confident and secure because of being
in his/her usual place of residence where family and friends can participate with skilled healthcare professionals in providing
care. A team of healthcare professionals together with the patient and his/her family, friends or caregiver collaborate to furnish
care to the patient under the supervision of a licensed physician.

Home health care includes skilled nursing, therapy, home health aide and social services provided to a patient on a one-on-one
basis. Different levels of treatment, training and education are customized to a patient's needs and may involve family, friends
and caregiver as necessary. Patients discharged to home from a healthcare facility is assured of continuity of skilled care.

| Who may receive Home Health Care services |

Anyone can qualify for home health care services as long as the criteria for admission to home health care services are met.
This will be determined by your physician and the skilled home health agency staff who will do an initial assessment.

| Who pays for Home Health Care services |
 |
|
Approved home health care services are covered 100% by Medicare and Medi-Cal. Private insurance coverage varies with individual
policies. Self pay or private pay are handled on a case to case basis. We process insurance verification, eligibility and submit
claims on the patient's behalf.
